New Patient InformationWe commit to providing quality, compassionate and professional care to your child(ren). It is our goal to provide the highest level of care to all children regardless of race, religion, sex, national origin, disability or insurance status. In order to maintain these standards, the following policies are in place to maintain the integrity of our interactions with you and your family. Please understand it is our desire to serve your needs to the best of our abilities and we need your assistance in developing a harmonious relationship. We encourage you to tell your child about their dental visit. We recommend that you refrain from using words like “shot,” “needle,” “drill,” “pull,” or “hurt.” You wouldn’t tell your child you are taking them to a movie and then say “it won’t hurt.” If you have dental fears, your child can easily perceive them and we would ask that you not discuss the appointment in great detail if that is the case. Let your child know that the doctor and staff need to count their teeth and everyone at the office can answer their questions for them when they arrive. The natural parent or legal guardian of the child must be present for the examination. We perform invasive procedures that require the consent of the legally authorized adult and your presence is required to do that. If you are not the natural parent of the child (i.e. foster parent, grandparent with custody, adoptive parent, etc.), you must provide us prior to or at the time of the initial appointment with documentation that establishes legal custody and your authorization to make healthcare decisions on behalf of the child. This documentation must come from the court or social services agency responsible for this determination.
